Supervise and train primary care providers (PCPs); Maximize core model execution, including clinical quality, efficiency, outcomes, and clinician/patient satisfaction; Provide direct patient care as a PCP and/or other clinical duties; Focus on PCP performance, engagement, and building clinical-operations synergy and culture; Assess, diagnose, treat, manage, educate, promote health, and coordinate care for patients with acute and complex chronic health needs; Lead care teams; Engage with transitional care teams and other resources for patients unable to come to the office; Lead Super Huddle (SH) and Transforming Care Meeting (TCM) weekly; Play an active role in center management and cover for other providers; Recruit patients and interview additional providers.
