Supervise and train primary care providers (PCPs) in assigned center. Maximize core model execution, improving clinical quality, efficiency, outcomes, and clinician/patient satisfaction. Provide direct patient care as a PCP and/or other clinical duties. Allocate time to leadership responsibilities, including PCP performance, engagement, and building strong clinical-operations synergy and culture. Independently provide care for patients with acute and chronic illnesses. Take full accountability for patient care and outcomes. Engage with hospitalists. Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation. Lead care team. Engage with transitional care team and other resources. Lead Super Huddle (SH) and Transforming Care Meeting (TCM) weekly. Play an active role in center management and cover for other providers. Recruit patients and interview additional providers. Manage, mentor, and coach PCPs. Perform other duties as assigned.
