Independently provides care for patients with acute and chronic illnesses. Takes full accountability for patient care and outcomes. Engages with hospitalists for patients in the hospital. Responsible for assessment, diagnosis, treatment, management, education, health promotion, and care coordination. Leads care team. Engages with transitional care team for patients unable to come to the office. Leads Super Huddle and Transforming Care Meeting weekly. Plays an active role in center management, recruiting patients, and interviewing providers. Manages, mentors, and coaches PCPs. Rounds with PCPs. Partners with Center Operations Director/Market General Manager to drive financial performance and increase membership.
