Supervises and trains primary care providers (PCPs). Maximizes core model execution, improving clinical quality, efficiency, outcomes, and satisfaction. Allocates time to direct patient care and leadership responsibilities. Manages PCP performance, engagement, and fosters clinical-operations synergy. Provides direct patient care for older adults with acute and chronic illnesses. Takes accountability for patient care and outcomes, seeking consultation when needed. Engages with hospitalists for patient care. Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation. Leads care teams (medical assistant, care facilitator, care coordinator). Manages patients in hospital, SNF, LTC, or homebound settings. Leads Super Huddle (SH) and Transforming Care Meeting (TCM) weekly. Plays an active role in center management, covering for other providers. Recruits patients and interviews additional providers. Manages, mentors, and coaches PCPs for outstanding clinical outcomes. Conducts leadership rounding with PCPs. Partners with Center Operations Director/Market General Manager to improve financial performance and increase membership. Performs other duties as assigned.
