Supervise and train primary care providers (PCPs) in assigned center. Maximize core model execution, including clinical quality, efficiency, outcomes, and clinician/patient satisfaction. Provide direct patient care as a PCP and/or other clinical duties. Allocate time to leadership responsibilities, including PCP performance, engagement, and building clinical-operations synergy and culture. Independently provide care for patients with acute and chronic illnesses. Take accountability for patient care and outcomes. Engage with hospitalists when patients are hospitalized. Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation. Lead care team. Engage with transitional care team for patients unable to come to the office. Lead Super Huddle (SH) and Transforming Care Meeting (TCM) weekly. Play an active role in center management and covering for other providers. Recruit patients for the center and interview additional providers. Manage, mentor, and coach PCPs. Partner with Center Operations Director/Market General Manager to drive financial performance and increase center membership.
