Supervise and train primary care providers (PCPs) in assigned center. Maximize overall core model execution, including improving clinical quality, efficiency, outcomes, and clinician/patient satisfaction. Accountable for overall clinical outcomes of assigned center. Allocate time to direct patient care as a PCP and/or other clinical duties. Allocate time to leadership responsibilities, including PCP performance, engagement, and building a strong clinical-operations synergy and culture. Provide care for patients with acute and chronic illnesses in older adult patients. Take full accountability for patient care and outcomes, seeking consultation from specialists when needed. Engage with hospitalists when patients are hospitalized. Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs. Lead care team (medical assistant, care facilitator, care coordinator) for patients able to come to the office. Engage with transitional care team and other resources for patients unable to come to the office. Lead Super Huddle (SH) and Transforming Care Meeting (TCM) weekly. Support Center Manager/Center General Manager and market clinical leader. Monitor daily center census. Play an active role in center management and cover for other providers. Recruit patients for the center and interview additional providers. Manage, mentor, and coach PCPs to deliver outstanding clinical outcomes. Perform other duties as assigned.
