AFH is a nonprofit Federally Qualified Health Center and certified Patient-Centered Medical Home whose mission is to eliminate disparities in access to health care for the underserved multi-ethnic diverse population of California and Nevada states. AFH’s services encompass general family practice, pediatric care, mental health, and behavioral health services as well as adult day health care services. We have served over 35,000 patients last year with over 95,000 visits across a dozen clinic locations. We are currently recruiting Pediatric Nurse Practitioner for one of our pediatric location. The Provider is responsible for the prevention, diagnosis, and treatment of patients within the scope of his/her field of training. Potential Provider is expected to be familiar with the mission of ALL FOR HEALTH, HEALTH FOR ALL and work collaboratively to achieve it. They must be cognizant of and comply with AFH policies and procedures as well as state and federal regulations. The provider will work with other staff as part of an interdisciplinary patient care team.
ESSENTIAL DUTIES
- Obtain comprehensive health histories from pediatric patients and their caregivers and perform age-appropriate physical examinations.
- Assess, diagnose, treat, and manage acute, chronic, and preventive healthcare needs for infants, children, and adolescents.
- Develop, discuss, and implement individualized treatment plans while educating patients and families on diagnoses, medications, preventive care, and healthy lifestyle practices.
- Prescribe medications and treatments in accordance with California law, clinical practice guidelines, and the standard of care.
- Order, interpret, and follow up on laboratory tests, diagnostic imaging, and other diagnostic studies.
- Review incoming reports (e.g., laboratory results, imaging studies, EKGs, and specialist consultations), document findings, and ensure appropriate follow-up.
- Order referrals to specialists and community resources when services are not available within the organization.
- Perform routine well-child examinations, developmental screenings, immunizations, and preventive health services according to current pediatric guidelines.
- Monitor growth and development and identify developmental, behavioral, and mental health concerns requiring intervention or referral.
- Comply with infection prevention and control policies and promote compliance throughout the clinic.
- Participate in quality improvement initiatives, peer review activities, Patient-Centered Medical Home (PCMH) meetings, provider meetings, and other required organizational meetings.
- Participate in daily care team huddles and pre-visit planning to support coordinated, patient-centered care.
- Collaborate effectively with physicians, nurses, medical assistants, behavioral health providers, and other interdisciplinary team members to ensure comprehensive patient care.
- Respect patients' and families' values, cultural backgrounds, and preferences, and actively engage them in shared decision-making.
- Perform on-call duties as assigned in accordance with organizational policies.
- Perform other duties as assigned.
QUALIFICATIONS
- Licensed to practice in California and maintain license through obtaining CMEs
- Current D.E.A. certificate
- Current ACLS certification
- Current CPR certification
- Able to manage multiple priorities
- Ability to speak, understand, write, and type English as well has have basic math and spelling skills
- Manual dexterity

Bilingual skills are preferred, but not necessary for this position. If an employee speaks a language other than English, they may be subject to proficiency testing.
