As a Community Health Worker at the Asian American Center of Frederick, you can help bring equity and access to community members who often face barriers. You will conduct outreach, prevention education, community engagement, standardized screening, care-barrier assessment, navigation, and care linkage across the Asian American Center of Frederick's community health programs. You will work across several grant-funded programs. Daily tasks could include: connecting families to pediatric care, WIC, Judy Centers, and FCPS Pre-K; and helping patients navigate cancer, diabetes, hypertension, and other chronic illnesses, including screening, prevention, and management. You will also play an integral role in connecting underserved populations with other appropriate community resources. Key Responsibilities:Our Ideal candidate* would have the following experience and skills:
MDH CHW certification, or eligible to certify within the onboarding period. (AACF can support certification as an MDH-accredited CHW Training Center.)
Comfortable with smartphones, tablets, and basic data entry.
Reliable transportation for community-based work.
Experience with health education and/or health promotion activities
Strong communication and interpersonal skills
Bilingual/multilingual abilities
Experience working with immigrant and refugee populations and understanding of the unique challenges they face
Strong knowledge of the healthcare industry
Knowledge of Frederick County resources and the Golden Mile / Frederick City communities.
Familiarity with local community resources and services for immigrants
Excellent organizational and time-management skills
Proficiency in Microsoft Office & Google Suite
Digital literacy, including the ability to use common computer applications and the propensity to learn and adopt new platforms quickly
A valid driver's license
A flexible schedule, some evenings and weekends will be required
Community-based role with regular travel across Frederick City and County, including events and partner sites.
Ability to lift light materials (e.g., outreach supplies) and work occasional evenings or weekends for events.
