Directly supervise and train primary care providers (PCPs) in the assigned center. Maximize core model execution, improving clinical quality, efficiency, outcomes, and clinician/patient satisfaction. Allocate time to direct patient care as a PCP and/or other clinical duties. Allocate remainder of time to leadership responsibilities, including PCP performance, engagement, and building strong clinical-operations synergy and culture. Independently provides care for patients with acute and chronic illnesses. Takes full accountability for patient care and outcomes. Engages with hospitalists. Responsible for assessment, diagnosis, treatment, management, education, health promotion, and care coordination. Leads care team. Engages with transitional care team. Leads Super Huddle (SH) and Transforming Care Meeting (TCM) weekly. Plays an active role in center management and helps cover for other providers. Manages, mentors, and coaches PCPs. Leadership rounding with PCPs. Partners with Center Operations Director/Market General Manager to drive financial performance and increase membership. Performs other duties as assigned.
