Supervise and train primary care providers (PCPs).
Maximize core model execution, improving clinical quality, efficiency, outcomes, and clinician/patient satisfaction.
Accountable for overall clinical outcomes of the assigned center.
Allocate time to direct patient care as a PCP and/or other clinical duties.
Focus on leadership responsibilities, including PCP performance, engagement, and building clinical-operations synergy and culture.
Assess, diagnose, treat, manage, educate, promote health, and coordinate care for patients with acute and complex chronic health needs.
Lead care teams.
Engage with transitional care teams and other resources for patients unable to come to the office.
Lead Super Huddle (SH) and Transforming Care Meeting (TCM) weekly.
Play an active role in center management and cover for other providers.
Recruit patients and interview additional providers.
Manage, mentor, and coach PCPs to deliver outstanding clinical outcomes.
Partner with Center Operations Director/Market General Manager to drive financial performance and increase center membership.
