Independently provides care for patients with acute and chronic illnesses encountered in older adult patients. Takes full accountability for patient care and outcomes. Engages with hospitalists when patients are hospitalized. Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation. Leads their care team. Engages with transitional care team and other resources for patients unable to come to the office. Leads Super Huddle (SH) and Transforming Care Meeting (TCM) weekly. Plays an active role in center management and helps cover for other providers. Manages, mentors, and coaches PCPs. Partners with Center Operations Director/Market General Manager to drive financial performance and increase center membership.
