Supervise and train primary care providers (PCPs).
Maximize core model execution, clinical quality, efficiency, outcomes, and clinician/patient satisfaction.
Accountable for overall clinical outcomes of the assigned center.
Allocate time to direct patient care as a PCP and/or other clinical duties.
Focus on leadership responsibilities, including PCP performance, engagement, and building clinical-operations synergy and culture.
Provide assessment, diagnosis, treatment, management, education, health promotion, and care coordination.
Lead care teams.
Engage with hospitalists and transitional care teams.
Lead Super Huddle (SH) and Transforming Care Meeting (TCM) weekly.
Play an active role in center management and provider recruitment.
Manage, mentor, and coach PCPs.
Partner with Center Operations Director/Market General Manager to improve financial performance and increase center membership.
