Supervise and train primary care providers (PCPs).
Maximize core model execution, clinical quality, efficiency, outcomes, and clinician/patient satisfaction.
Provide direct patient care as a PCP and/or other clinical duties.
Allocate time to leadership responsibilities, including PCP performance, engagement, and building clinical-operations synergy and culture.
Assess, diagnose, treat, manage, educate, promote health, and coordinate care for patients with acute and complex chronic health needs.
Lead care teams.
Engage with transitional care teams and other resources for patients unable to come to the office.
Lead Super Huddle (SH) and Transforming Care Meeting (TCM) weekly.
Play an active role in center management and cover for other providers.
Recruit patients and interview additional providers.
Manage, mentor, and coach PCPs to deliver outstanding clinical outcomes.
Conduct leadership rounding with PCPs.
Partner with Center Operations Director/Market General Manager to improve financial performance and increase center membership.
Perform other duties as assigned.
