Directly supervise and train primary care providers (PCPs) in the assigned center. Maximize core model execution, including improving clinical quality, efficiency, outcomes, and clinician/patient satisfaction. Allocate time to direct patient care as a PCP and/or other clinical duties. Focus on leadership responsibilities, including PCP performance, engagement, and building clinical-operations synergy and culture. Provide assessment, diagnosis, treatment, management, education, health promotion, and care coordination for patients. Lead care teams. Engage with hospitalists and transitional care teams. Lead Super Huddle and Transforming Care Meeting. Play an active role in center management, recruiting patients and providers. Manage, mentor, and coach PCPs. Partner with Center Operations Director/Market General Manager to improve financial performance and increase membership.
