Schedule: 12/hr and 10hrs shifts available; days, nights, weekends

LPN Pay Range: $28 - $30/hr Depending on experience; night differential available as well

RN Pay Range: $37 - $45/hr Depending on experience; night differential available as well

Job Duties Include:

• Provide holistic care to the individuals, their family members and communities/population, per plan of care
• Complete assessments (Initial and ongoing) and care plans
• Create education and individualized care plans
• Collaborate with the interdisciplinary health care team and other providers as needed
• Develop, coordinate and update individualized plan of care based on evidence in collaboration with the individual's family, interdisciplinary team, and medical home providers to promote continuity of care across the continuum.
• Accurate, thorough and timely documentation
• Attend and Participate in meetings and conferences

Requirements Include for RNs:

• Associates Degree in Nursing or higher
• Current NYS RN License and Registration
• OPWDD experience is a plus
• 2+ years of Nursing experience preferred
• Current Basic Life Support Certification
• Experience working with individuals with intellectual or physical disabilities a plus
• Experience with SigmaCare and/or E-Clinical Works a plus

Requirements include for LPNs:

• Graduate of an approved LPN Program
• Current NYS LPN License and Registration
• OPWDD experience is a plus
• 2+ years of Nursing experience preferred
