Onsite near Diamond Springs, CA
Pay Range: $62+/hour (depending on experience)

• Full 2024-2025 School Year
• Dates: ASAP Start - 6/17/25
• Part Time position
• Mod-Severe disabilities on caseload
• K-12th grade age groups, exact size/ages TBD
• Itinerant position (mileage can be reimbursed between school sites)

Responsibilities:

• Conduct comprehensive evaluations of students to assess their physical abilities and limitations.
• Develop individualized treatment plans based on evaluation results and collaborate with the school team to integrate therapy into the student's educational program.
• Provide therapy services to students in individual and group settings.
• Monitor and document progress, modify treatment plans as necessary, and communicate regularly with the school team and parents/guardians.
• Provide consultation and training to teachers and other school staff to support the integration of therapy goals into the classroom and daily routines.
• Participate in IEP meetings and assist in the development of appropriate goals and objectives.
• Maintain accurate and complete records, including progress notes, reports, and billing documentation.

Requirements:

• Graduate degree in Physical Therapy from an accredited program.
• Valid CaliforniaPhysical Therapy license and certification from the National Board for Certification
• Must be able to pass a background check
