A facility located in Coos Bay, OR is currently seeking a Registered Nurse Case Manager to join their team full time!Check out the details below:

Title: Registered Nurse Case Manager
Job Type: Permanent / Direct Hire
Setting: Hospice
Location: Coos Bay, OR
Schedule: Full Time, 40hrs/wk, Mon-Fri or Tues-Sat, occasional on-call
Pay:Starting pay is $45/hr and goes up with experience, up to $7,000 sign on bonus and relocation assistance available!

The Registered Nurse Case Manager duties to include but not limited to:
-Perform all nursing functions for an assigned caseload of terminally ill patients, coordinates the activity of multiple internal care team disciplines, and collaborates with external medical community partners
-Review documentation for all new patients assigned, and communicate with admissions staff, or other members of the Care Team, as appropriate to ensure continuity of care
- Must have ability to work in diverse and unpredictable surroundings in personal residences, nursing homes, assisted living facilities, adult foster homes
- Promote and coordinate multidisciplinary care provided in an interdisciplinary fashion, engaging with other professional care team members as needed to provide full and effective service in the physical, emotional, social and spiritual dimensions of care

REQUIREMENTS:
-Current RN License in Oregon required
-CPR and BLS required
- Hospice experience is required
- Must be able to meet physical demands (lifting a max of 50 Ibs)
-Must be willing/able to drive in a variety of road/weather conditions
