The Community Health Center of Franklin County is seeking a motivated and experienced Nurse Practitioner or Physician Assistant to join our primary care practice. The Nurse Practitioner / Physician Assistant will play a key role in delivering comprehensive healthcare services to patients of all ages. This position offers the opportunity to work collaboratively with physicians and other healthcare professionals to provide personalized and patient-centered care.
Key Responsibilities:
- Conduct comprehensive health assessments, including medical history, physical exams, and diagnostic tests, to evaluate patients' health status.
- Formulate and implement evidence-based treatment plans in collaboration with physicians, focusing on preventive care, acute illness management, and chronic disease management.
- Order and interpret diagnostic tests, such as laboratory tests and imaging studies, to assist in diagnosis and treatment planning.
- Prescribe medications and treatments according to established protocols and guidelines, monitoring patient response and adjusting treatment as necessary.
- Provide patient education and counseling on health promotion, disease prevention, medication adherence, and self-management strategies.
- Perform procedures within scope of practice, such as wound care, suturing, and joint injections, as appropriate.
- Document patient encounters and treatment plans accurately and efficiently in electronic medical records.
- Collaborate with interdisciplinary team members, including nurses, medical assistants, and specialists, to coordinate patient care and ensure continuity of care.
Qualifications:
- Nurse Practitioner: Master's or Doctoral degree in nursing from an accredited program, with certification as a Nurse Practitioner in [specialty].
- Physician Assistant: Master's degree in physician assistant studies from an accredited program, with certification as a Physician Assistant.
- Current license to practice as a Nurse Practitioner or Physician Assistant in the state of [location].
- 1-3 years of experience working as a Nurse Practitioner or Physician Assistant in a primary care setting preferred.
- Strong clinical assessment and diagnostic skills, with the ability to formulate accurate diagnoses and develop appropriate treatment plans.

- Proficiency in using electronic medical records and other clinical software.
