Qualifications
Physical Therapist current license in the state of Michigan
Valid driver's license and current auto insurance
Current CPR certification
Must be familiar with Medicare guidelines and regulations for home health
Current TB, HEP and CPR
Must be familiar with elderly rehab in community / home setting
Ability to use our electronic medical records system
Responsibilities
Part time and PRN positions are available leading to full time position
Providing skilled -OT/ PT services.- Admit Clients / Evaluations / Treatments
Coordinating care with main office
Communication with physician, office and other clinical staff
Supervision if needed
Providing screenings and assessments when needed
Must be very well organized and have good customer service skills - Must have good communication skills
