Job Description:
Job Responsibilities:
- Reach out to obtain and verify consumer information for assessment appointments.
- Schedule assessments efficiently, meeting productivity goals and documenting all outreach activities.
- Ensure the right resources are assigned for each assessment and address consumer needs during scheduling.
- Maintain an efficient scheduling process and identify areas for improvement.
- Assist with scheduling in RN appointments when possible.
- Respond to inquiries and provide information about the program and its services.
- Offer case management support and coordinate care plans.
- Identify and refer appropriate community resources to support care plans.
- Follow up to ensure consumers access the resources outlined in their care plans.
- Adhere to all project-related policies, procedures, and performance standards.
- Perform additional duties as assigned by management.
Required Education:
- Bachelors Degree from an accredited institution.
Preferred Education:
- Bachelors Degree in Social Work or a related mental health field.
Required Experience:
- 1-3 years of telephone-based case management experience.
Preferred Experience:
- Experience with pediatric psychiatric or medically fragile patients.
Additional Information:
- Location: Remote with occasional on-site requirements.
