Job title: Case Manager
Pay: $25-28
Location: Berkley, CA and Oakland, CA
Conduct outreach to eligible patients via phone and in-person at various sites, building relationships and assessing needs.
Engage patients and caregivers in care plan development, aligning services with patient goals.
Facilitate referrals, assist with applications, and provide health education (e.g., harm reduction, disease prevention).
Maintain caseload per program standards, using data tools to track outcomes and meet grant deliverables.
Assist patients with public benefits (Medi-Cal, SSI/SSDI, housing) and access to medical/behavioral health services.
Collaborate with care teams, external partners, and patient supports, ensuring ongoing communication.
Provide case management to patients with complex medical or behavioral conditions (e.g., HIV/AIDS, homelessness).
Participate in team meetings, crisis interventions, and quality improvement initiatives.
Mentor and coach case managers, interns, and volunteers; provide onboarding and technical support.
Document services in required systems (EHR, HMIS), ensuring compliance with policies.
Qualifications:
High School diploma or GED.
3+ years of experience in community health or social work, or 1+ year as a Case Manager II/III, with certification as a Certified Alcohol and Drug Counselor.
Proficient in Microsoft Office, data management, and project coordination.
Reliable transportation, with valid license and insurance.
Experience in training, coaching, and meeting program goals.
