Title: Family Nurse Practitioner
Job Type: Permanent / Direct Hire
Setting: Wellness clinic
Location: Phoenix, AZ
Schedule: Monday- Friday 8a-5p
Pay: Competitive pay based on experience plus a full benefits package

The Family Nurse Practitioner responsibilities include but are not limited to:
-Deliver primary care services, with a client-centered, harm-reduction focus.
-Medically Assisted Treatment (MAT) and administration of medications such as – Psychiatric, Vivitrol, Suboxone or Subutex, etc.
-Refer patients to specialists, hospitals, community agencies or in-house staff as needed.
-Maintaining accurate and timely records, writing reports, support letters; writing chart summaries for transfer or legal purposes as required.
-Providing emergency care (e.g. initiating CPR), treatment of anaphylaxis, acute asthma or other conditions as required.
-Ensure that a medical evaluation, including a medical history and physical examination, has been performed at admission and annually.
-Completes physical examinations, procedures lab specimens and records the results of the test, presents medical

REQUIREMENTS:
-Master’s degree in nursing from an accredited college or university
-Clean and active APRN license in the state of Arizona
-3 years’ experience as a NP in medication assisted treatment and family medicine
-AHCCCS Medicaid Provider Access ID required
-CPR certified within 3 months of hire
