Responsibilities for bilingual nurse

• Conduct scripted and unscripted health coaching conversations with patients taking into account their psychological profile
• Adherence to the general HealthForward inc.  processes,   program specific processes
• Oversee and supervise the care being provided to the patient by Licensed Practical Nurses (LPNs) and Paraprofessionals through regular home visits
• Act as the coordinator of care for the Multidisciplinary team by providing care to patients
• Transport and utilize required medical equipment and supplies
• Reports any changes in the client’s condition to the Clinical Manager and physician
• Work under the direction of the Director, maneuvers multiple tasks independently with a fast-paced proactive vs
• Applies advanced knowledge to conduct multispecialty subjective assessments to evaluate the skills of nurses supporting the TRICARE - Global Nurse Advice Line (GNAL)
• Clearly and completely document audit findings within the audit tracking tool
• Provide information and guidance and care coordination to the patient for effective care transition, improving self-management skills, promoting post hospital discharge follow up and maintaining patient safety in the community

Qualifications for bilingual nurse

• SPECIALTY AREAS - If no qualified candidate, will be willing to train
• Must have passed RN licensing exam and currently in good standing with the appropriate state Department of Health Professions
• Minimum of 2 years of previous RN experience required
• Must be willing to travel to Washington DC, Greenbelt, Prince Frederick, Rockville, Silver Spring and Waldorf
• Associates (ASN) or Bachelors degree (BSN) with an active license in the state of practice
