Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.
This role involves serving as a Sterile Compounding Pharmacist focused on home infusion therapy. The position requires immediate availability and offers a 13-week contract assignment in Plano, TX.
Key Responsibilities:
- Evaluate patients’ eligibility for home infusion therapy and coordinate care plans.
- Develop, monitor, and adjust individualized medication therapies based on clinical assessments, lab values, and pharmacokinetics.
- Prepare, verify, and dispense sterile medications adhering to USP standards as well as state and federal regulations.
- Educate patients, caregivers, and healthcare providers about medication regimens and infusion administration.
- Collaborate with healthcare teams to ensure patient safety and continuity of care.
- Manage infusion pumps and related equipment, maintaining quality assurance and adherence to safety protocols.
- Provide drug information, maintain accurate records, and support regulatory compliance and medication safety initiatives.
Qualifications and Desired Experience:
- Doctor of Pharmacy (PharmD) or graduate of an accredited School of Pharmacy.
- Active, unrestricted Pharmacist license in Texas.
- Minimum of one year in sterile IV compounding practice, or equivalent experience such as:
- Experience with non-sterile compounding, immunizations, and Medication Therapy Management (MTM).
- Completion of an ASHP-accredited Pharmacy Residency.
- Completion of an Advanced Pharmacy Practice Experience (APPE).
- Strong clinical knowledge of home infusion pharmacy and ability to interpret laboratory data.
- Excellent communication, patient counseling, and customer service skills.
