Schedule: Monday–Friday, 8:00 AM–5:00 PM
Hours: 40 hours not guaranteed
On-Call: 1 week/month, after hours
License: VA RN License (Compact accepted)
Productivity: 30 points/week
Mileage: $0.43/mile
Counties: Patrick, Henry, Pittsylvania & Franklin

Job Overview

We are seeking an experienced Registered Nurse (RN) to provide comprehensive assessments, care coordination, and case management for high-risk patients. The RN will develop individualized care plans, coordinate with providers and community resources, and help ensure patients receive appropriate, cost-effective care.

Key Responsibilities

• Perform comprehensive assessments and develop individualized care plans.
• Coordinate care with physicians, social workers, discharge planners, and other providers.
• Manage care transitions and connect patients with medical and non-medical resources.
• Monitor clinical, functional, and financial outcomes.
• Identify opportunities for health promotion and disease prevention.
• Advocate for patients while maintaining privacy and confidentiality.
• Provide timely intervention to prevent or minimize adverse outcomes.

Requirements

• Active, unrestricted VA RN License; Compact accepted
• Associate or Bachelor's degree in Nursing or related field
• Home Health experience required
• Experience with HCHB and OASIS
• Experience with wound care, wound vacs, IVs, and lab draws
• Strong clinical assessment, critical thinking, and care coordination skills
• Case Management certification preferred/required
• Ability to meet 30-point weekly productivity requirement
