POSITION SUMMARY
This position plays a critical role in developing and maintaining strong community partnerships with health plans, healthcare providers, schools, housing agencies, behavioral health organizations, community collaboratives, and social service partners to support coordinated systems of care and improve outcomes for participants. The ECM Program Manager supports staff through reflective supervision, coaching, training, and operational leadership while ensuring compliance with agency, county, state, and managed care requirements. The position may also maintain a limited caseload to support continuity of services, staff development, and program operations.
ESSENTIAL FUNCTIONS
RESPONSIBILITIES
Responsible for developing, maintaining and implementing program policies and procedures and ensure that program staff adhere to program and Agency policies and procedures.
Accountable for the recruitment of direct reports and participate in the interviewing process for potential staff, interns and/or volunteers.
Responsible for the development, implementation, and oversight of Enhanced Care Management services in alignment with Wellness Collaborative requirements.
Provide leadership and supervision to ECM staff, including Lead Care Managers and/or Housing Navigation staff.
Attend supervisor meetings and collaboration meetings with San Diego Wellness Collaborative.
Ensure quality, timely, and compliant completion of assessments, care plans, progress notes, referrals, and all required documentation.
Review and approve individualized care plans and related ECM documentation.
Support outreach and engagement efforts for high-need and hard-to-reach populations experiencing complex behavioral health, medical, housing, and social challenges.
Develop and maintain strong collaborative partnerships with community organizations, healthcare providers, school districts, housing agencies, behavioral health systems, managed care plans, and other stakeholders.
Represent Harmonium within community collaboratives, Health Plan meetings, county initiatives, and interdisciplinary care coordination meetings.
Assist in developing workflows, procedures, and training materials that support effective ECM service delivery and compliance.
Monitor program performance metrics, outcomes, and quality improvement initiatives to ensure achievement of program goals and contractual expectations.
Participate in care coordination activities and may carry a limited caseload as needed to support program operations and continuity of care.
Ensure services are delivered using trauma-informed, strength-based, culturally responsive, and person-centered approaches.
Support staff in identifying and connecting participants to community resources, including medical care, behavioral health services, housing supports, transportation, food access, educational services, and social supports.
Responsible for ensuring personnel issues are proactively addressed in accordance with agency policies, procedures, and timelines.
Attend required staff meetings, trainings, community meetings, and collaborative partnerships.
JOB COMPLEXITY
Work effectively across multiple systems including healthcare, behavioral health, education, housing, and social services to support coordinated participant care.
Under HR’s direction will participate in preparing and reviewing in any legal related matters.
Responsible for developing trainings in collaboration with the Learning & Development Committee to ensure trainings reflect agency core values and initiatives.
