LPN Clinical Coordinator (Full-Time, Exempt)

Falcons Landing is seeking a motivated, experienced, and compassionate LPN Clinical Coordinator to join our Healthcare team. This is an excellent opportunity for a skilled nurse ready to take on a leadership role within a highly respected, not-for-profit Life Plan Community.

Key Responsibilities

• Oversee daily clinical operations within the Memory Care Unit
• Supervise and support nursing staff, including hiring, training, and performance management
• Ensure compliance with Virginia Department of Social Services (DDS) regulations
• Coordinate care delivery and maintain accurate clinical documentation using EMR systems
• Promote a positive, collaborative, and high-performing team environment

Qualifications

• Active Licensed Practical Nurse (LPN) license in Virginia required
• Prior leadership or management experience in a healthcare setting preferred
• Experience in assisted living, memory care, or long-term care
• Familiarity with PCC and electronic medical records (EMR) systems
• Proficiency in Microsoft Office applications
• Strong organizational, communication, and time management skills
