Position Summary

The Licensed Vocational Nurse (LVN) provides direct nursing care and health services to participants enrolled in the Community-Based Adult Services (CBAS) program under the direction of a Registered Nurse (RN) and in accordance with physician orders, California Title 22 regulations, the California Board of Vocational Nursing and Psychiatric Technicians (BVNPT), CBAS program requirements, and organizational policies.

The LVN assists in implementing individualized care plans, administers medications and treatments, monitors participants' health status, documents nursing care, and collaborates with the interdisciplinary team to promote participant independence, safety, and quality of life.

Essential Duties and Responsibilities

Direct Participant Care

• Provide skilled nursing care within the LVN scope of practice.
• Monitor participant health status and report significant changes to the RN or physician.
• Obtain and document vital signs, weights, blood glucose levels, oxygen saturation, and other clinical measurements.
• Assist participants with activities of daily living as needed.
• Observe participants for physical, emotional, cognitive, and behavioral changes.
• Respond appropriately to participant illnesses and emergencies.

Medication Administration

• Administer prescribed medications in accordance with physician orders and California nursing regulations.
• Perform the Five Rights of Medication Administration.
• Monitor participants for medication effectiveness and adverse reactions.
• Document medication administration accurately in the Medication Administration Record (MAR).
• Report medication discrepancies and errors immediately according to facility policy.
• Maintain proper medication storage, security, and inventory.

Treatments and Clinical Procedures

Perform nursing procedures within the LVN scope of practice, including:

• Wound care as ordered
• Blood glucose monitoring
• Dressing changes
• Skin assessments
• Catheter care (as authorized)
• Oxygen administration
• Nebulizer treatments
• Infection control procedures
• First aid and emergency interventions

Participant Monitoring

Report abnormal findings promptly to the supervising RN.

Documentation

Maintain accurate and timely documentation including:

• Nursing notes
• Medication administration records (MAR)
• Treatment records
• Vital signs
• Blood glucose logs
• Incident documentation
• Participant observations
• Physician communications
• Daily clinical records

Ensure documentation complies with:

• California Title 22 regulations
• CBAS Program Requirements
• HIPAA
• Organizational documentation standards

Care Plan Implementation

• Implement individualized nursing care plans developed by the RN.
• Assist the RN with participant assessments.
• Observe participant progress toward established goals.
• Report participant responses to treatments and interventions.
• Participate in interdisciplinary team meetings as requested.

Infection Prevention

• Follow all infection prevention and control procedures.
• Utilize standard and transmission-based precautions.
• Wear appropriate Personal Protective Equipment (PPE).
• Monitor participants for signs and symptoms of infectious diseases.
• Report communicable illnesses according to facility policy.
• Maintain a clean and safe clinical environment.

Participant and Family Education

Reinforce education provided by the RN regarding:

• Medication compliance
• Disease management
• Nutrition
• Infection prevention
• Fall prevention
• Personal hygiene
• Healthy lifestyle practices

Refer complex educational needs to the supervising RN.

Emergency Response

• Recognize changes in participant condition requiring immediate intervention.
• Initiate emergency procedures when necessary.
• Provide Basic Life Support (BLS) within certification.
• Assist emergency medical personnel.
• Complete required incident documentation following emergencies.

Maintain compliance with:
