Supervise and train primary care providers (PCPs) in assigned center. Maximize overall core model execution, including improving clinical quality, efficiency, outcomes, and clinician/patient satisfaction. Allocate time to direct patient care as a PCP and/or other clinical duties. Allocate time to leadership responsibilities, including PCP performance, engagement, and building strong clinical-operations synergy and culture. Independently provides care for patients with acute and chronic illnesses. Takes full accountability for patient care and outcomes. Engages with the hospitalist whenever one of their patients is in the hospital. Responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation. Leads their care team. Engages with the transitional care team and others. Leads Super Huddle (SH) and Transforming Care Meeting (TCM) weekly. Plays an active role in the management of their center and helps cover for other providers. Manages, mentors and coaches PCPs. Leadership rounding with the PCPs. Partnering with Center Operations Director/Market General Manager to drive continued improvement of center financial performance, and helping increase center membership. Performs other duties as assigned.
