Provides spiritual care and counseling for patients and their family members. Completes an assessment of the patient's and family's spiritual needs at the time of admission and throughout course of care. Coordinates with the patient/family and interdisciplinary team to establish, review and revise the plan of care for each patient. Administers and coordinates the administration of sacraments.
