Supervise and train primary care providers (PCPs) in assigned center. Maximize core model execution, improving clinical quality, efficiency, outcomes, and clinician/patient satisfaction. Allocate time to direct patient care as a PCP and/or other clinical duties. Allocate remainder of time to leadership responsibilities, including PCP performance, engagement, and building clinical-operations synergy and culture. Assess, diagnose, treat, manage, educate, promote health, and coordinate care for patients with acute and complex chronic health needs. Lead care team. Engage with hospitalists. Lead Super Huddle (SH) and Transforming Care Meeting (TCM) weekly. Play active role in center management and cover for other providers. Recruit patients and interview additional providers. Manage, mentor, and coach PCPs. Conduct leadership rounding. Partner with Center Operations Director/Market General Manager to improve financial performance and increase membership.
