Independently provides care for patients with acute and chronic illnesses. Takes full accountability for patient care and outcomes. Engages with the hospitalist whenever one of their patients is in the hospital. Responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination. Leads their care team. Engages with the transitional care team and others for patients unable to come to the office. Leads Super Huddle (SH) and Transforming Care Meeting (TCM) weekly. Plays an active role in the management of their center and helps cover for other providers. Manages, mentors and coaches PCPs. Leadership rounding with the PCPs. Partnering with Center Operations Director/Market General Manager to drive continued improvement of center financial performance.
