Independently provides care for patients with acute and chronic illnesses encountered in older adult patients. Takes full accountability for patient care and outcomes. Engages with hospitalists for patient care. Responsible for assessment, diagnosis, treatment, management, education, health promotion, and care coordination. Leads care team. Plays an active role in center management, recruiting patients, and interviewing providers. Manages, mentors, and coaches PCPs.
