Independently assess acute and non-acute clinical problems. Perform and document physical assessments and patient histories. Analyze trends in patient conditions and develop, document, and implement patient management plans. Plan patient care based on in-depth knowledge of the specific patient population and/or protocol. Anticipate and identify physiological and/or psychological problems. Serve as a patient advocate. Write admission, transfer, and discharge orders. Order and interpret appropriate laboratory and diagnostic studies. Order appropriate medication and treatments. Refer patients for consultation when indicated. Document through in-depth progress notes and summaries. May perform invasive procedures independently upon completion of documented competency. Participate in patient care rounds and conferences. Communicate patient management strategies to members of the patient care team. Collaborate with members of the multidisciplinary team. Recognize situations requiring immediate physician attention and initiate life-saving procedures when necessary. Use advanced communication skills to problem-solve complex situations and improve processes and service to patients. Collaborate with other multidisciplinary team members to analyze and evaluate current systems of health care delivery. Participate in outside activities that enhance personal and professional growth and development. Initiate arrangements and write orders for SNF discharges and complete appropriate paperwork. Work collaboratively with physicians, nurses, PT, social workers, family, and key caregivers to transition the patient to a lower level of care as soon as medically appropriate. Introduce self to patient/family and explain nurse practitioner role. Facilitate patient/family conferences to review treatment goals, optimize resource utilization, provide family education, and identify post-SNF needs. Enhance a collaborative relationship to maximize the patient’s/family’s ability to make informed decisions regarding goals of care, palliative care, and hospice. Facilitate discharge to appropriate level of care and use preferred providers when additional services are required. Prescribe medication to patients based on State of practice.
