Independently provides care for patients with acute and chronic illnesses. Takes full accountability for patient care and outcomes. Engages with hospitalists. Responsible for assessment, diagnosis, treatment, management, education, health promotion, and care coordination. Leads their care team. Engages with transitional care team and other resources for patients unable to come to the office. Leads Super Huddle (SH) and Transforming Care Meeting (TCM) weekly. Plays an active role in center management and covering for other providers. Manages, mentors, and coaches PCPs. Leadership rounding with PCPs. Partnering with Center Operations Director/Market General Manager to drive financial performance and increase center membership.
