Provides physical therapy services according to a written physician's plan of care. Advises, consults, and teaches appropriate treatment procedures and supportive activities to clients, family members, and other agency staff. Assists the provider in evaluating level of function and participates in the development of the plan of care. Prepares clinical and progress notes, translates exercises into functional activities, and develops plans for modifying equipment. Confers regularly with the attending provider regarding client's progress and interprets social and physical factors in the environment affecting care. Provides families with information, support, and encouragement. Prepares and submits clinical and progress summaries. Supervises physical therapy assistants. Maintains accurate clinical records.
