Serves as caregiver to the resident during the scheduled work period in long term care. Provides resident-centered nursing care and daily living assistance to assigned resident under the supervision of a registered nurse (RN). Delivers age-appropriate care related to the physical and psychological needs of the resident as per care plan. Provides assistance with basic health care needs including daily living activities (bathing, toileting, grooming, dressing/undressing), obtaining and recording vital signs, and providing psychosocial support. Assists with resident transfers, repositioning, and walking. Completes tasks related to food preparation and dining. Assists residents with meals and snacks. Aids providers and nursing staff members with procedures. Documents resident interactions and outcomes. Communicates resident's changing condition to the charge nurse. Collects non-invasive body fluid specimens and gathers vital signs. Provides housekeeping and laundry services. Provides assistance and documentation with wellness and recreational programs.
