Support the PCP in the delivery of care by gathering and documenting relevant clinical data, as appropriate and per patient need. Assists in specialty office procedures as directed. Participate in the daily huddle to identify individual patient needs for upcoming appointments. Clinical liaison between patient and provider for questions, concerns and testing results. Communicate effectively with other members of the care team in coordinating the individualized plan of care. Support PCP during in office procedures and examinations. Provide outreach and education to patients in need of services as identified through quality improvement initiatives (Population Health Management). Perform duties within scope of practice to the highest level of licensure. Provide direct support & guidance to other members of the care team as appropriate. Able to perform assessment data collection on all patients utilizing tools / forms in EMR. Adequately assess and reassess pain. Educate the patient and family regarding pain management. Provide age appropriate care to patients in the community setting. Consistently document procedures completed in the EMR, to include medications administered and follow-up assessments. Deliver medication utilizing the five (5) rights and observes patient for desired untoward effects. Maintain current knowledge of medications and their correct administration. Establish priorities of patient care based on needs and office resources. Demonstrates the ability to assist providers with procedures and perform services requiring technical and manual skill. Perform other duties as required.
