Job Summary EGA Associates is seeking a qualified Physician Director to provide clinical leadership and medical oversight at a Primary Care Community Based Outpatient Clinic serving Veterans primarily residing in and around Pierre, South Dakota. The clinic provides primary care services in an outpatient clinical environment. Essential Functions - Serve as the clinic's Physician Director and provide medical leadership and oversight for covered clinical services. - Oversee the quality, appropriateness, and coordination of medical care provided to assigned and unassigned patients. - Support compliance with applicable contract requirements, VA clinical standards, credentialing requirements, and professional standards of care. - Provide clinical guidance and leadership to members of the Patient Aligned Care Team. - Collaborate with clinic leadership, healthcare professionals, VA personnel, and telehealth-based mental health staff. - Review available clinical, quality, access, and performance data with the care team. - Lead or support continuous improvement activities involving critical clinical and team-based care processes. - Promote effective care coordination, communication, delegation, and escalation among members of the interdisciplinary care team. - Support same-day access to face-to-face and virtual care during regular clinic operating hours. - Coordinate with available support services, which may include pharmacy, laboratory, and diagnostic imaging. - Identify and escalate patient safety, quality-of-care, credentialing, or clinical performance concerns when appropriate. - Participate in quality assurance, peer review, clinical audits, and other oversight activities required by EGA or the VA. - Maintain all required documentation, licenses, certifications, credentials, and clinical privileges. When credentialed and privileged to function as a Primary Care Provider, the physician will also: - Provide primary care services to assigned and unassigned patients presenting for care. - Deliver care consistent with the physician's licensure, clinical privileges, prescriptive authority, and approved scope of practice. - Assess, diagnose, treat, and manage acute and chronic medical conditions. - Ensure each patient's care plan includes appropriate medical recommendations for clinically indicated care. - Offer clinically indicated healthcare services to patients assigned to the PACT. - Provide or arrange for care to which the patient consents. - Develop personal health plans in collaboration with patients and PACT staff. - Incorporate appropriate care management and care coordination into each patient's treatment plan. - Order and interpret clinically appropriate diagnostic studies, laboratory testing, medications, consultations, and referrals. - Provide leadership to the PACT, including the appropriate delegation of care activities and processes to qualified team members. - Maintain accurate, timely, and complete medical documentation in accordance with VA and clinic requirements. - Perform additional duties as assigned.
