Responsibilities
Provide evidence-based physical therapy services to home health patients on-site.
Develop and implement personalized care plans, updating progress weekly.
Assess, measure, and document patient outcomes to demonstrate progress.
Adhere to regulatory standards, documentation, and privacy requirements.
Demonstrate reliability by meeting visit targets and documenting results in the system.
Educate patients and caregivers on exercises, safety, and home program adherence.
Collaborate with cross-functional team for coordinated care and referrals.
Maintain accurate visit notes, treatment logs, and billing-related documentation.
Participate in ongoing professional development to advance clinical competencies.
Utilize data and pipelines to identify high-potential patient cases and outreach.
Proactively identify opportunities for service expansion and process improvements.
Report on territory activity and outcomes to support data-driven decisions.
Experience/Qualifications
Licensed physical therapist with current state license
Home health PT experience delivering evidence-based care
Accurate documentation of visits, outcomes, and billing
Regulatory compliance and privacy adherence (HIPAA, etc.)
Develop and update personalized home care plans weekly
Strong reliability to meet visit targets and show progress
Use data pipelines to identify high-potential cases and outreach
Patient and caregiver education on exercises and safety
Collaborate with cross-functional team for coordinated care
Participation in ongoing professional development and clinical advancement
