Provide direct nursing care in accordance with established policies, procedures and protocols. Coordinate unit activities and assure accountability of all assigned tasks. Instruct and educate patients and family members. Administer prescribed medications, change dressings, clean wounds, and monitor vital signs. Monitor, record and communicate patient conditions as appropriate utilizing computerized documentation systems. Serve as the primary coordinator of all disciplines for well-coordinated patient care. Coordinate patient's discharge planning needs with members of the healthcare team.
