Job: Transitions of Care Pharmacist

Pay: $65-$68/hr- DOE

Location: Sylmar, CA

Setting: Hospital

Schedule: Shift Varies, Monday - Friday 8am - 5pm

Rx Relief is a leader in Pharmacy recruiting and is looking for a Transitions of Care Pharmacist to join our team in Sylmar. Rx relief has great entry-level Outpatient and Inpatient Hospital opportunities!

Transitions of Care Pharmacist Responsibilities:

• Review Perscriptions
• IV Experience is preferred
• Organize & Label Medications
• Provide and Counsel Patients

Transitions of Care Pharmacist Job Requirements:

• An active California Pharmacist license is required
• Must have Pharm D
• 1+ year clinical Pharmacist experience

Rx relief Hiring Requirements:

• Must be 18+ years old
• Background and Drug Screening required
• Health Screening required
• Must be able to provide proof of eligibility to work in the US
