Soliant is seeking a Pharmacy Specialist to join our team!

Location: Berkeley, CA

Sign-On Bonus: $10,000

Position: Pharmacy Specialist

Duties:

• Initiate, modify, and discontinue medications within a Collaborative Practice Agreement
• Focus on management of plex disease states to help patients reach optimal therapeutic goals
• Simplify patient medication regimens, minimize the risk of harmful medication interactions, and educate patients to enhance adherence and clinical outes
• Responsible for policy development, continuous process improvement, continuing education, clinical and medical staff development, and Pharmacy APPE student and resident preceptorship
• Perform patient care assessments, order and interpret laboratory tests, prescribe, modify, and discontinue medication therapies
• Monitor clinical outes, municate with medical providers, perform electronic documentation, and refer patients to other healthcare providers

Schedule: Monday to Friday, day shifts

Ideal Candidate:

• PharmD
• PGYI + PGYII or 4+ years of current ambulatory care oncology experience
• BCOP preferred
• Ability to obtain CA pharmacy license
