Soliant is seeking a highly skilled and motivated Advanced Practice Pharmacist with Oncology experience for a direct hire/perm opportunity in a Clinic setting in California.This direct hire position offers a sign on bonus!Must have an active California license and the required experience to be considered for this role.

Position: Advanced Practice Pharmacist - Oncology Clinic

Schedule: Monday - Friday, Day Shifts
Key Responsibilities:

• Initiate, modify, and discontinue medications within a Collaborative Practice Agreement.
• Manage plex disease states to help patients achieve optimal therapeutic goals.
• Simplify patient medication regimens, minimize the risk of harmful interactions, and educate patients to enhance adherence and clinical outes.
• Develop policies, improve continuous processes, provide continuing education, and contribute to clinical and medical staff development.
• Serve as a preceptor for Pharmacy APPE students and residents.
• Perform patient care assessments, order and interpret laboratory tests, and prescribe, modify, and discontinue medication therapies.
• Monitor clinical outes, municate with medical providers, perform electronic documentation, and refer patients to other healthcare providers.

• Doctor of Pharmacy (PharmD) degree.
• pletion of PGYI + PGYII or 4+ years of current ambulatory care oncology experience.
• Board Certified Oncology Pharmacist (BCOP) preferred
• Active/clear California pharmacist license
