Our company is currently seeking a Primary Care Provider to join our team. Our clinics focus on primary care and work with a group of specialist to provide complete care for all of our patients. We are focusing on quality care and provide a workplace which our health team enjoy coming to work each day for patient care.

Responsibilities

• Well visits: Well Woman Exams, Complete Annual Physicals, AWV
• Same day appointments (fever, UTI, headache, allergies, etc)
• Chronic disease management (diabetes, hypertension, COPD, etc.)
• Preventive care, healthy living, disease prevention
• Ordering Diagnostic Testing & Labs as Needed
• Ability to Understand and Work with Specified Care Flows of Treatment
• Ability to use EHR System Proficiently & Complete Notes in a Timely Manner
• Ability to see 10-15 Patients/Day in Clinic

Requirements :

• AZ Licensed Provider (NP/PA)
• DEA License (AZ) Without Restrictions
• No History of Malpractice
• No restrictions with Credentialing with Medicare/medicaid/Commercial Insurances
