Community Home Health Services, a division of Medical Services of America, Inc., currently seeks an experienced Full-Time Patient Care Coordinator - Non Clinical for our Home Health office in West Palm Beach (Palm Beach, Broward) FL, Ft. Lauderdale (Broward) FL.
This person will be responsible for the effective intake, education and coordination of home health and/or hospice services and all ancillary medical services. Serves as a liaison between the Agency and the medical community to coordinate care.
· Acts as a liaison between field staff and physicians in obtaining plans of care and appropriate modified orders
· Serves as an educational resource to hospitals, physician offices, clients and other health care facilities.
· Coordinates care with all rehab personnel (PT, OT, & ST) and obtains appropriate orders for care.
· Delivers orders to physicians for signature, provides feedback and updates physicians on the patient’s condition.
Qualifications and Skills
· Three to five years’ experience promoting community awareness as it relates to home health and/or hospice strongly preferred.
· Reliable transportation and company required auto liability insurance.
