We are seeking a highly skilled and experienced Pharmacist to join our team in Yuma, AZ. This is a decentralized role in a hospital setting, where you will play a critical role in patient care and be actively involved in various clinical activities.

Experience:

• Must have at least 2 years of hospital pharmacy experience in a facility of similar size (250 census) within the last 3 years.
• Experience with Vanco AUC dosing is required.
• IV/pounding experience is highly desirable.

Technology:

• Familiarity with EPIC, Lexip, Baxter IQ, and Omnicell Cabinets.

Key Responsibilities:

• Attend and participate in patient care rounds, providing medication-related insights and remendations.
• Manage and monitor patients' medication therapy, including pharmacokinetic dosing and adjustments.
• Participate in code blue and rapid response teams, ensuring timely medication administration.
• Provide education and counseling to patients and healthcare professionals on proper medication use.
• Prepare and pound sterile IV medications in pliance with hospital standards.

Qualifications:

• PharmD required.
• Active Arizona pharmacist license required.
• Strong clinical pharmacy skills with a focus on patient care and safety.
• Excellent munication and collaboration skills with a multidisciplinary healthcare team.
