Duties:
As a Transition of Care Pharmacist, you will:

• Function as a drug specialist and therapy consultant to promote optimum medication use.
• pound, package, label, and dispense drugs and intravenous admixture solutions.
• Review and confirm the accuracy of work performed by pharmacy technicians.
• Review prescribed drug therapy for appropriateness, effectiveness, and safety based on specific patient data such as age, disease state, and clinical status.
• Perform quality assurance, drug use evaluation, and adverse reaction reporting activities.

pensation: Starts at $70-$120/hr

Schedule: Day-shifts

Qualifications:

• PharmD degree
• Licensed Pharmacist in CA
• 1+ years of experience
