We are currently seeking a compassionate and experienced Licensed Vocational Nurse (LVN) to join our team in Union City, CA. The selected candidate will provide vital support to an elementary student with specific 1:1 care needs. This role requires prior school experience and proficiency in assisting students with various medical needs, including but not limited to Diabetes management, G-Tube feeding, and Tracheostomy care.

Responsibilities:

• Provide general LVN duties while offering personalized 1:1 care to support the elementary student's medical needs throughout the school day.
• Assist the student with activities of daily living, medication administration, and other healthcare procedures as required.
• Monitor and manage the student's condition, including Diabetes management, G-Tube feeding, and Tracheostomy care, adhering to established protocols and physician instructions.
• Collaborate with school staff, healthcare professionals, and the student's family to ensure a holistic approach to care and support the student's educational and medical needs.
• Maintain accurate documentation of interventions, observations, and communication regarding the student's health status.

Needed Credentials:

• Licensed LVN in the state of California.
• Demonstrated experience in providing care to individuals with Diabetes, G-Tube feeding, and Tracheostomy care.
• Prior experience in a school setting is highly preferred.
• Strong communication and interpersonal skills, with the ability to collaborate effectively with a multidisciplinary team.
