Company DescriptionHealthCare Support is actively seeking a Dietitian to fill an opening with a major Healthcare organization in Bronx, NY.
Daily Responsibilities Dietitian:
- Assessment and Care Planning: Conduct initial and follow-up face-to-face and telehealth assessments, evaluate participants' nutritional status, and develop individualized nutrition care plans based on guidelines and professional knowledge.
- Therapeutic Diets and Recommendations: Evaluate physical conditions, calculate therapeutic diets, and recommend care plans to the team, ensuring to document diet orders, modifications, and participant preferences.
- Clinical Documentation and Compliance: Maintain up-to-date participant files, document changes in conditions, and ensure compliance with agency standards through detailed PACE documentation.
- Interdisciplinary Coordination and Monitoring: Work with the interdisciplinary team to integrate and monitor nutrition care plans, providing ongoing progress updates and adjusting plans as necessary.
- Education and Professional Development: Provide nutrition counseling, participate in education programs, and stay updated with professional certifications and industry trends.
- Shift: 9am - 5pm (1 hour Break)
Required Qualifications for Dietitian:
Education: Bachelors Degree from accredited Dietetics program
- A minimum of one (1) year of experience in clinical nutrition.
- Minimum of one (1) year of experience working with a frail or elderly population or, if the individual has less than one (1) year of experience but meets all other requirements, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
- Home Care experience, preferred.
- License:
- Current registration to practice as a Registered Dietitian through the Commission on Dietetic Registration – Registered Dietitian Nutritionist (RDN).
- New York State license: Certified Dietitian Nutritionist (CDN).
